Make sure you have Adobe Acrobat Reader.

Click on the red paper icon.

A new window will open and in a few seconds (based on your connection speed) a PDF file will appear.

Go the FILE / SAVE AS /
Find a place on your computer and rename the application however you want.

OR

Go to the disk icon in Acrobat
CLICK it and save the file. You can rename it here too.

Close the window and open the application on your computer to fill it out. When you are ready SAVE it with the changes.

To email it back - simply attach to your email to
